Our aspiration jointly with our children centre partners under the umbrella of Birmingham Forward Steps is to give the children and families living in the Ladywood and Perry Barr Districts the very best start in life.
The successful applicant will work as part of a diverse Health Visiting team led by supportive District Clinical Team Managers and supported by District Practice Leads. The teams include Assistant Practitioners who support the team to deliver specific care pathways, community staff nurses who focus on supporting the teams to deliver care to children with medical needs. Working in close liaison with our early years colleagues the teams have access to family support and other supportive roles within the early years teams. We plan to develop new roles and services that are tailored to the meet the needs of local families.
The post holder will carry continuing responsibilities for caseload management and the assessment of care needs as part of the service offer for health visiting to include Community, Universal, Universal Plus and Universal Partnership Plus services. This will involve being responsible for the delivery of the Healthy Child Programme and the development, implementation and evaluation of programmes of care which are sensitive, relevant and responsive to the diverse needs of individual and targeted patients/clients, families, groups and communities. The post holder will also contribute to the Public Health Agenda to meet Public Health targets and address health inequalities
