Summary 9 months fixed term post An exciting opportunity has arisen to play a leading role in the development of a new service aimed at improving the way elderly patients are discharged from hospital. Working for Wiltshire Care Partnerships – the Wiltshire Care Home representative body – you will work alongside health, social care and Care Home providers to shape the service. About the role We are looking for several people to create a team of assessors who will work with local care home managers, Wiltshire Council and the Wiltshire CCG to develop a system of Trusted Assessments in the south of the county. The role of the Trusted Assessor is to undertake an independent and unbiased assessment to determine suitability and requirements on behalf of care providers. You will be making a significant contribution to the development and shaping of a service whose aim is to improve the hospital discharge process for elderly patients. Some evening and weekend work will be required as appropriate to the discharge process being supported. About you A background in health or social care is not essential. Your interpersonal skills are the primary consideration as the role involves meeting families and working with them in a sensitive and effective way. The successful candidate will need to develop relationships with providers and work alongside partners and people involved in discharge planning, have an ability to remain neutral and work as part of a multi-disciplinary and multi-organisational team. You must have the ability to work in a fast-paced environment and be able to manage multiple planned and unplanned tasks. Shortlisting: Monday 4th November Interview: Monday 11th November
