Home Dialysis CSW

Job description

EAST AND NORTH HERTFORDSHIRE NHS TRUST

Renal Department

Post: Clinical Support Worker Higher Level – Band 3

Location: Renal Home Therapies

Hours: 37.5 hrs per week

East and North Hertfordshire NHS Trust provides long term Home haemodialysis and peritoneal dialysis for patients across a network that spans 5 sites including the parent unit situated at the Lister Hospital in Stevenage, with 4 satellite units covering Hertfordshire, Bedfordshire and West Essex. The 5 units currently provide in-centre haemodialysis for approximately 500 patients and 78,500 sessions per annum.

In recent years there has been a growing recognition that self-care in the home either by haemodialysis or peritoneal dialysis should be encouraged wherever possible to enhance health and quality of life prospects. There is also recognition that significant improvement in the experience of patients receiving ICHD can be gained by sharing tasks associated with their treatment with the nursing staff – so-called Shared Care. While still remaining in-centre, some patients can contribute significantly to their care and some can become wholly self-caring. The clinical service delivery model is therefore changing from traditional dependency towards patient training, empowerment and support to deliver care which is customised to patient choice, clinical need and ability.

This is an excellent opportunity within the Renal Home Therapies Department at East and North Herts NHS Trust. We are seeking to recruit a motivated, collaborative, dynamic, and innovative Clinical Support Worker Higher Level, Band 3, to support and be part of our continuous growing expansion of the provision of home therapies service.

Applicants for these posts must have successfully completed part 1 and 2 of the Renal Unit Training Programme and candidates will have to demonstrate a flexible approach to work, excellent attendance and a commitment to complete a qualification at level 3 as determined by the East and North Herts NHS Trust.

Duties will include supporting in delivery of the renal units’ strategy which promotes, engages and successfully achieves self-care in 15 % of patients on RRT. Have commitment in achieving targets in numbers of patients on the Home Therapies model of care through supporting education both in centre and in the outreach setting including in their place of residence (home/care home/prison/self-care area).
