Reablement Coordinator

Job description          

To enable people living in Cheshire West and Chester to be supported by their family and friends and rebuild strong community networks. Effectively lead a group of reablement workers to deliver a high quality reablement service with the aim of promoting and sustaining people’s independence living in the community. To liaise with the Rapid/Community Reablement Manager and all members of the team to ensure that service delivery throughout the reablement pathway is well coordinated and complies with organisational policies, procedures and Care Quality Commission regulated guidance. To ensure that the reablement service achieves the best quality outcomes for service users, their carer’s and families. An exciting opportunity has arisen to join the Rapid/Community Reablement Service within the Cheshire West and Chester Council. The service has recently undergone a significant redesign and is based at Civic Way in Ellesmere Port: We have a one permanent & one temporary position on offer for a Reablement Coordinator Grade 7 - 32 hours per week (0.87FTE). £24,313– £26,999 pro-rata, per annum On top of your basic pay you shall also receive enhancements, travel time & mileage claims. The successful candidate will manage a team of Reablement Workers in a specific area however shall be expected to cover the whole footprint of CWaC. You will be responsible for achieving the best outcomes for all CWaC customers. Supporting, enabling people and vulnerable adults to maintain their own wellbeing and independence within their own home in the community. The main objective of the work we do is to put the service user at the centre of every conversation. You will be able to demonstrate a track record in motivating, leading, managing and developing staff, alongside a clear understanding of leadership responsibilities in social care and have excellent communication/interpersonal skills, be open and willing to adopt different ways of working and demonstrate the desired THRIVE behaviours and culture, which can be evidenced in line with the performance ratings. Support managers to deliver social care services in line with the budget and performance targets; manage staffing budgets reducing the levels of staff absences and deliver best value for money across all of our services. Utilising community and digital resources to maximise their social capital in line with the requirements of the Care Act 2014.
